How to Obtain a Copy of Your Birth Certificate

If a birth occurred within the Town of Smithtown, a certified copy may be obtained from the Registrar located in the Town Clerk's Office.

A certified copy of a birth certificate may be issued only to:

  • The person named on the birth certificate, if 18 years of age or older
  • The parents of the person named on the birth certificate
  • The lawful representative of the person named or the parents of the person named on the birth certificate
  • A municipal, state or federal agency when needed for official purpose
  • A person with a New York State Court Order

Required Identification

  • Driver’s License
  • Military ID
  • Non-Driver's ID
  • Passport
  • Police Report of Lost or Stolen ID

If the applicant’s last name on the identification differs from the information on the certificate, a copy of a marriage license or proof of a legal name change must be provided.

Note: Failure to include the required identification and necessary documentation will result in rejection of your application.

A request can be made in person or to request by mail, fill out the application form, enclose a copy of one of the acceptable forms of identification and necessary documentation along with a money order made payable to the Town of Smithtown.

Fee - $10 per transcript

Note: Personal checks are not accepted. We accept cash, credit and debit at the counter. An additional convenience fee will be applied to all credit / debit transactions.
